Survey on Primary Care Renewal

I received an invitation today for our group to participate in a survey on Primary Care Renewal


"The Primary Care Renewal Project Team at the Department of Health and Wellness and Health PEI invites healthcare professionals, social and community programs, and non-profit organizations to participate in a survey to inform and support Primary Care Renewal initiatives on PEI, including the development of Patient Medical Homes and Neighbourhoods.

 

Please designate one person to complete the survey on behalf of your organization or group. The survey will be open until Friday, October 13th, at 4 p.m. AST."


I'd love to receive your comments or input on any ideas you'd like the Department of Health and Wellness and Health PEI to consider and include for autistic people for their Primary Care Renewal initiatives on PEI and the development of Patient Medical Homes and Neighbourhoods. I will include those in our response so don't hold back. Overview of the initiative below:


"Overview 

The Primary Care Road Map and Primary Care Renewal Projects were launched in 2021 by the Department of Health and Wellness and Health PEI.  These projects have focused on the development of Patient Medical Homes and Neighborhoods as well as other initiatives, with the goal of providing quality, equitable, and patient-focused primary care services on Prince Edward Island.  Project initiatives have fallen under four priority areas, which are access to care, team-based care, innovation and technology, and enhanced and integrated care.

 

A Patient Medical Home (PMH) is defined by the College of Family Physicians of Canada (CFPC) as “…a family practice defined by its patients as the place they feel most comfortable - most at home - to present and discuss their personal and family health and medical concerns…”.  PMHs take a multidisciplinary approach to primary care with teams that consisting of physicians, nurse practitioners, medical secretaries, and a combination of other providers such as registered nurses, licensed practical nurses, social workers, and allied health professionals. PMHs are a team-based care model with the goal of providing patient-centered care that provides the right care by the right provider in a timely manner.

 

Patient Medical Neighbourhoods (PMN) are a set of relationships that revolve around the patient and the patient's PMH and are not necessarily geographic constructs.  They consist of a network of care involving multiple health care providers and services outside of the clinic.  This set of relationships includes many realms of health care outside of primary care, such as: 

·        Medical specialists 

·        Acute care 

·        Long-term care 

·        Home care 

·        Mental Health and Addictions 

·        Community pharmacists 

·        Other health care providers 

·        Broader social and community supports (i.e. Social Services, NGOs, etc.) 

·        Etc.

 

The services and organizations in the PMN are already providing high quality services to clients within the community. Within the PMN and Community, the Patient Medical Home partners with other providers to coordinate, collaborate, and share responsibilities in order to provide comprehensive care for patients (College of Family Physicians of Canada, 2020). 

Engagement has been and will continue to be an important component of implementing these initiatives. 

The purpose of this engagement is as follows:  

·        Support an understanding of various programs, professions, and organizations roles related to the health and wellbeing of Islanders. 

·        Gain a better understanding of where services are provided across PEI.

·        Engage partners in conversations around neighborhood services and how these services align and support the work within the Patient Medical Home. 

·        Seek advice on how to establish ongoing engagement as PMH and PMN develop. 

·        Seek advice on how to build pathways between primary care providers, patients, and established services and organization within the Patient Medical Neighbourhood to improve health and wellbeing of Islanders. "

I got a response! The survey is quite different than I was expecting.


"The survey should take you up to 15 minutes (hopefully). However, it depends on how much information you want to provide about your organization (some may only put a few words while others go into great detail).

 

The survey is not the kind where you can save your answers and come back later continue where you left off. I have listed the questions below if you would like to review them before launching the survey.
