Letter of Invitation to a Primary Care Renewal Engagement Meeting - help wanted

Further to our participation in the Survey on Primary Care Renewal, PEI Autistic Adults received a Letter of Invitation to a Primary Care Renewal Engagement Meeting. This is a three hour meeting to be held in Charlottetown in January. I would love to hear from our members with any suggestions of an "autistic" way of participating in this meeting because right now I can't imagine networking with these people for three hours. At the same time I'd really really really like 'the development of Patient Medical Homes and Neighbourhoods on Prince Edward Island' to be done with consideration for autistic adults and autistic seniors. So any and all suggestions are very welcome.

 

"The Primary Care Renewal (PCR) Project Team (Health PEI and the Department of Health and Wellness) is hosting an engagement meeting with community partners seeking advice on the development of Patient Medical Homes and Neighbourhoods on Prince Edward Island. This session will be focused on relationship building, information sharing, and exploring opportunities for strengthening pathways between primary care and community partners to address Islander's social determinants of health. This letter is an invitation to participate in an engagement meeting hosted by the Primary Care Renewal (PCR) Project Team.

The PCR Roadmap (Health PEI and the Department of Health and Wellness) has introduced Patient Medical Homes and Neighbourhoods to PEI’s primary care system. The Patient’s Medical Home (PMH) is the vision for the future of family practice in Canada, endorsed by the College of Family Physicians of Canada (CFPC). The PMH emphasizes the role of family practice and team-based care in providing high-quality, compassionate, and timely care. The concept of the Patient’s Medical Neighbourhood (PMN) takes the idea of team-based care in the PMH further to describe and reinforce a network of care involving multiple providers, services, and resources such as hospitals, medical specialists, community pharmacies, community health providers, social resources, and organizations that address the social determinants of health. 


The PCR Project Team is seeking advice from community partners on PEI to inform the development of the Patient Medical Homes and Neighbourhoods. The session's objectives are:

• To share information about Primary Care Renewal, PMH, and PMN progress to date.

• To provide an opportunity for community partners to network and share information about their organization, program, or service and learn about other services available on PEI.

• To provide a forum to share potential opportunities and barriers related to the building of partnerships between PMH, PMN, and community partners.

• To identify opportunities to strengthen pathways between primary care and community partners to address Islander's social determinants of health.

• To lay the foundation for future action items and ongoing relationship building."
